What Are The Requirements?
  • BMI must be 30 or over.
  • BMI can be 27 or over if you have an underlying health issue such as; high cholesterol, high blood pressure, or diabetes.
  • If you are transferring from another provider, your BMI must have initially been 28 or higher
  • Must have lab work, including your CBC, CMP, and TSH from the past 12 months available. (Or it can be ordered by our providers)
